A Publication of the National Council on Administrative Fellowships (NCAF) August 2022 2021 - 2022 NCAF Benchmarking Survey Executive Summaries 1 National Council on Administrative Fellowships 2021-2022 Benchmarking Surveys Summary Introduction The National Council on Administrative Fellowships (NCAF) is a premier program of the National Center for Healthcare Leadership (NCHL) that brings both healthcare management graduate programs and administrative fellowships sites together to build a national pipeline of future healthcare leaders. NCAF members consist of 83 fellowship sites and 40 graduate programs. The direction of NCAF is monitored and set by the NCAF Steering Committee, which includes the work of the Benchmarking, Research, and Metrics Workgroup and the Emerging Leaders Workgroup. In addition to shared benchmarking, members participate in the centralized application service to recruit fellows, discussion boards, and educational opportunities. All members agree to abide by the guidelines of the NCAF Code of Good Practice, which includes upholding uniform application dates and helps to coordinate the recruitment and offer process across fellowship programs. Overview The National Council on Administrative Fellowships (NCAF) completed the 5 th annual benchmarking survey for students and applicants and the 7 th annual benchmarking survey for fellowship sites, graduate programs, and fellows after a two-year hiatus during the COVID-19 pandemic. Over 480 hospitals, health systems, students, graduate programs, and administrative fellows responded to these surveys between January and March of 2022. Respondents provided recruitment data, anecdotal feedback on the early careerist experience and career trajectory, Centralized Application Service (NAFCAS) satisfaction, and information fellowship opportunities and graduate programs. This data collection process is designed to monitor dynamics within the field of administrative fellowships and healthcare management education, as well as the impact of NCAF activity on outcomes. Fellowship Sites (Sites) – Described as respondents to the annual benchmarking survey that are healthcare organizations (hospitals, health systems, etc.) that sponsor at least one administrative fellowship for early careerists. 2 National Council on Administrative Fellowships (NCAF) – As a premier program of the National Center for Healthcare Leadership, NCAF is leading the way across the country to build the bridge between practice and academia. It is made up of fellowship sites and graduate health management programs that are working to continuously improve early-career development opportunities for our next generation healthcare leaders. NCAF creates an industry standard in the recruitment for administrative fellows. In doing so, NCAF allows for a uniform and coordinated recruitment process and helps organizations access a national pipeline of the next generation of healthcare industry leaders. NAFCAS (National Council on Administrative Fellowships Centralized Application Service) – This is the one stop shop for students interested in fellowship sites. It provides a seamless way to upload transcripts, personal statement(s), resume, and letter of recommendation. This tool allows for fellowship sites to post opportunities for early careerists graduating from health management programs, organize top applicants, and communicate clearly and directly with candidates. National Center for Healthcare Leadership (NCHL) – Dedicated to advancing healthcare leadership and organizational excellence, NCHL hosts three premier programs under its scope of work, the National Council on Administrative Fellowships, Leadership Excellence Networks (LENS), and US Cooperative for International Patient Programs (USCIPP). January and March of 2022. The following data is designed to provide results on the dynamics within the field of administrative fellowship recruitment. spans the entire recruitment process from the organization’s point of view. From the decision to recruit through fellowship structure and outcomes for participants. Fellowship Recruitment Data The 2021 – 2022 fellowship site benchmarking survey received 62 responses from hospitals and health systems across the country. For the 2021 – 2022 recruitment cycle, 98% of the responding organizations accepted applications for their administrative fellowship program between June 2021 and January 2022. Of these, a majority recruited using the centralized application service, NAFCAS, and another process (69%) or solely through NAFCAS (30%). For the 2021 – 2022 recruitment cycle, the 62 respondents recruited for a total of 168 fellowship slots. Sites received an average of 70 applications per fellowship slot. Due to ongoing concerns of the COVID- 19 pandemic, many organizations conducted virtual interviews. A total of 1,584 students were interviewed by phone or other virtual methods. On average each organization held 4 in-person interviews per available fellowship slot. Organizations made more offers (191) than slots available (168) due to students dropping out of the interview process. A total of 140 students pulled out of the fellowship recruitment process prior to the offer period. Of the applicants reported by respondents, 1,487 completed a first-round interview. 32.5% of those completed a second-round interview, and only 5% of those completed a third or final round interview. After interviews, organizations ultimately made offers to 191 applicants to fill these roles. Sites reported that the current fellows at their organizations had various types of graduate degrees. Reportedly, master’s degrees were obtained degrees in Health Management (104) Business Administration (54), and Public Health (46). Of the fellows completing their fellowship in the summer of 2022, 85% were hired on into post-fellowship roles. Of the 62 sites, 55 intend on recruiting in the 2022 – 2023 cycle providing 147 fellowship slots. Applicant Focused Data Fellowship sites were surveyed on a variety of applicant specific topics. The top competencies desired of applicants included: • Initiative • Analytical Thinking • Communication Skills (Speaking/Facilitating) • Collaboration • Self-Awareness When reviewing administrative fellowship applications, fellowship sites noted the following aspects of an applicant as most important: • Ability to speak to previous projects or accomplishments • Communication • Critical Thinking • Previous work experience or internship • Diverse background 6 NAFCAS Satisfaction Fellowship sites were asked to rate their satisfaction of recruitment methods, specifically NAFCAS versus traditional methods (career websites, mail-in applications, etc.) In quality of applicants, application process, and offer acceptance process, NAFCAS had higher satisfaction rates compared to traditional methods. 41% responded that their organization is highly satisfied with both the quality of applicants and application process. Conclusion From this data, we see that hospitals and health systems continue to provide administrative fellowship programs to early careerists. Fellowship opportunities continue to remain competitive, with sites engaging with applicants with specific competencies. Respondents provided data on graduating class size, career trajectory of graduates, anecdotal feedback on NCAF and National Council on Administrative Fellowships Centralized Application Service (NAFCAS). This data collection process is designed to monitor dynamics of the early careerist experience, with a specific highlight on administrative fellowships, as well as the impact of NCAF activity on outcomes for students and graduate programs. The following data includes the demographics of responding graduate programs and the graduate program faculty experience through the recruitment process for administrative fellowships. NCAF has collected data on interview outcomes as well as anecdotal feedback on the recruitment process from both student and faculty perspective. Programmatic Data 37 health management graduate programs responded to the 2021 – 2022 benchmarking survey. Of these programs, 68% were National Council on Administrative Fellowship members and 94% were accredited by the Commission on Accreditation of Healthcare Management Education (CAHME). Between 2019 and 2022, median program size of health management programs grew from 20 to 25. Graduating class sizes for 2022 ranged from 4 to 80 students. Administrative Fellowship Process Of the reported students that graduated in 2022, 38% pursued fellowships during the 2021 – 2022 recruitment cycle compared to 49% of students in 2019. 70% of these students received at least one offer for an administrative fellowship position. The percentage of students receiving at least one fellowship increased 5% between 2019 and 2022. Responding programs reported a total of 1,098 graduating students in 2022. Of these students, 417 students pursued fellowships in the 2021 – 2022 cycle. These statistics show that the number of early careerists entering the healthcare workforce is continuing to rise, though not all are pursuing an administrative fellowship directly after graduation. NCAF members receive two fee waivers each administrative fellowship recruitment cycle. Of the responding graduate programs that were NCAF members, 48% provided both NAFCAS fee waivers to their students. In the search for administrative fellowships, NAFCAS is one of the most referenced resources. Graduate programs refer students to: •ACHE Post-Graduate Fellowship Listing •Hospital/Health System Careers Page •LinkedIn •NAHSE Website 9 NCAF surveys collected the same data from students, which showed that although the exact order varied, the top four resources used by students and referred by programs were the same. Graduate programs reported the median salary for students that accepted fellowships after graduation. The average of these salaries was $62,718. NAFCAS Satisfaction Graduate programs reported higher satisfaction rates for quality of administrative fellowship sites (NCAF = 4.73, non-NCAF = 4.64) and application process timeline (NCAF = 4.44, non-NCAF = 4.13) for the centralized application service, NAFCAS, compared to non-NCAF fellowship sites. Graduate programs were asked about the advantages to applying to NCAF-member fellowship sites. The top two results were the searchable directory of fellowships (NAFCAS) and the ease of application process for their students. Graduate programs were also asked about the perceived disadvantages to applying to NCAF- member fellowship sites. 14 graduate programs noted no perceived disadvantages. The remaining programs noted either cost of application (15 programs), timeline or timing of applications (11 programs), or limited number of fellowship opportunities (9 programs) as perceived disadvantages from the student perspective. Conclusion From this data, we see that the number of early careerists entering the workforce is on the rise, while trends show us that students are pursuing full-time jobs as well as administrative fellowships. NAFCAS data continues to show us that both students and faculty find the centralized application service a valuable tool in finding and selecting administrative fellowships. Data from 235 students was collected between January and March of 2022. Respondents provided recruitment data, anecdotal feedback on NCAF, satisfaction on National Council on Administrative Fellowships Centralized Application Service (NAFCAS), and the fellowship interviewing process. This data collection process is designed to monitor dynamics within the field of administrative fellowships, as well as the impact of NCAF activity on outcomes. The following data spans the entire recruitment process from the student’s career trajectory and fellowship application process. Fellowship Application Process For the 2021 – 2022 cycle, 91% of surveyed students applied for administrative fellowships. In their fellowship search, students identified several important factors when deciding to pursue a fellowship site. The top factors included: •Opportunity to Stay at the Organization •Mentorship Opportunities •Opportunity to Get a Wide Variety of Experiences When applying to fellowships, 72% utilized both NAFCAS and traditional methods (mail in, career pages, etc.). Applicants submitted a median of 8 applications, which one hour was spent on each. Applications submitted through NAFCAS took less time to complete compared to traditional methods (62.5 minutes versus 66.7 minutes). Students were asked about the various resources utilized in their search for fellowships. The top four indicated by students were: •Hospital & Health System Career Page •LinkedIn •Information Session •ACHE Post-Graduate Fellowship Listing Of those surveyed, 78% received a fellowship offer. Of these, 90% accepted their offer. The students that did not accept their offer chose to accept a full-time job offer instead. NAFCAS Satisfaction Students were asked about their satisfaction with the centralized application service, NAFCAS. Trended data shows us that student’s satisfaction has increased each year. On a scale from 1 to 5, satisfaction with the quality of sites increased from 4.28 to 4.42. Satisfaction with the quantity of sites increased from 3.86 to 3.98 and satisfaction with the application process increased from 3.92 to 4.09. Overall, student satisfaction with the centralized application service has increased over time, with the annual review of the code of good practice and addition of the several new fellowship sites each year. 12 Perception of Fellowships Respondents were asked about their perceptions and the current state of administrative fellowships. 89% of students consider administrative fellowships an opportunity for career advancement. At the same time, 53% of students considered other post-graduate opportunities in lieu of an administrative fellowship. Though fellowships are still widely popular among early careerists, we see graduating students also pursuing full time jobs, additional graduate school, internships, and part-time work opportunities. Demographics Students that participated in the 2021 – 2022 benchmarking survey were diverse across age, gender, and race/ethnicity. 67% respondents were between the ages of 18 and 24. A majority of responding students were White (161), with Asian students (71), Black or African American students (47), and Hispanic or Latino students (31) rounding out the remainder of the respondent group. 67% of students self-identified as female, 32% as male, 1 student identified as genderqueer, and 2 students declined to answer. Conclusion From this data, we see that students continue to find administrative fellowships to be a valuable post- graduate opportunity. The graduating classes are increasingly diverse and looking for diverse opportunities to pursue. Students utilize several resources to find and chose administrative fellowships with NAFCAS being top of the list. Fellows Feedback on Graduate Programs Fellows were asked a variety of questions around their fellowship program, including areas most and least prepared for. Most fellows participated in a program that was partially or fully projected based. 50% of responding fellows reported that their organization recruits between 1 and 3 fellows annually. During their fellowship, responding fellows worked approximately 45 hours per week, with 30% of those hours being remote. In their program, each fellow has the unique opportunity to work in a variety of roles and departments. In 2022, 67% of fellows reported that they will be completing the full-term of their fellowship as expected. At the time of survey, 55% intended on staying at their organization for a post-fellowship position, while 34% did not yet accept a post-fellowship position. Trended data shows the salary of administrative fellows increasing over time. (2018: between $65,000 and $99,999). Perception of Fellowships The fellows benchmarking survey asked about their perceptions of fellowships. 97% of fellows believe that fellowships are a valuable opportunity for career advancement. Fellows reported a net promoter score of 40, confirming that 54% of fellows would be promoters of administrative fellowships. Conclusion Fellows continue to receive high quality education from health management programs across the U.S as they enter leadership roles in hospitals and health systems. Data shows that fellowship sites continue to recruit new fellows to provide insight into various departments of the organization.
